* High school and middle school level, pre-participation sports physicals are not available in Kansas. In order to provide the best quality care, it is recommended that patients bring in a copy of their vaccination and immunization records, as well as a copy of their medical history to the Take Care Clinic in order to receive a school or sports physical.

If your insurance covers your visit, a one-time, easy copayment is required on the day you visit. If you do not have insurance, or prefer to pay out-of-pocket, we accept cash, credit, or debit cards. Sorry, no personal checks.
